First steps toward integrated diabetes care
Diabetes WA has secured a WAPHA grant to:
• Scope and profile diabetes services providing care for consumers living with type 2 diabetes in regional or metropolitan Western Australia aged 17 years and above
• Identify agreed priority areas for integration and potential areas for partnerships
• Codesign an innovative integrated diabetes service that addresses at least one of these priority areas.
We are reaching out to General Practitioners in different ways to discover your current situation, how you can be more supported to manage your patients with more complex type 2 diabetes, your experience of the current patient journey and ways to improve care connection.
Your participation in this survey is completely voluntary and responses will remain anonymous. Data collected through this survey may be used in publications on this important topic, in the hope that others can learn from the information gathered. Completion and submission of the online survey will be taken as your implied consent to participate.
